We want to support people who are at risk of developing Type 2 diabetes to live a healthier lifestyle.
When people are diagnosed with diabetes, we want to make sure they have enough information and support to help them live well with their condition. This could be through community groups, online support or access to a diabetes specialist in the community or in a hospital.
If your GP identifies you as being at risk of developing Type 2 diabetes, they may offer you the opportunity of taking part in the Healthier You: NHS Diabetes Prevention Programme. The course, which is currently delivered remotely via telephone or MS Teams conference calls, comprises group sessions covering nutrition, exercise and suggested behavioural changes to help participants achieve and maintain a healthy weight and become more physically active. There is also a one-to-one digital option to support those people at risk of Type 2 diabetes which is app/computer based.

The Diabetes service at Gloucestershire Hospitals NHS Foundation Trust supports patients with Type 1 and Type 2 diabetes in the county to manage their condition.
Patients with Type 2 diabetes are also supported in the community by the Community Diabetes Service at Gloucestershire Health and Care NHS Foundation Trust. Their team is made up of specialist nurses, dietitians and diabetes educators/facilitators.
The NHS Type 2 Diabetes Path to Remission Programme is a free programme to help people with Type 2 diabetes reduce their medications, lose weight, improve their health and potentially put their Type 2 diabetes into remission.
